This article covers using forms and checklists in the Storypark for Educators iOS and Android apps. For a full guide including form creation and admin management, see Educators: Forms and checklists.
Accessing forms and checklists on the app
Forms and checklists is available in the Storypark for Educators app on both iOS and Android. Once your service has forms and checklists enabled, you'll find it in the main navigation of the app.
To access it:
iOS: Tap the menu icon at the top left of the screen and select Forms from the sidebar
Android: Tap the menu icon at the top left of the screen and select Forms from the sidebar
This will open your service's list of published forms — any form that an Admin has published is available here for you to complete.
Note: If you don't see Forms in the app menu, your service may not yet have the feature enabled. Check with your service Admin or contact Storypark support.
Completing a form or checklist
To complete a form on the app:
Navigate to Forms from the app menu
Tap the form you want to complete
Work through each field — tap a checklist item to tick it, and tap a text field to type your response using the on-screen keyboard
Tap Submit when you've finished
Your submission is recorded with your name and the date and time it was completed.
Checklist items
Tap any checklist item to mark it as done. Tap it again to untick it. There's no requirement to complete every item before submitting — leave any items unticked if they're not applicable.
Text fields
Tap into a text field to bring up the keyboard and type your response. You can use voice dictation on both iOS and Android if you prefer to speak your response rather than type it.
Submitting
When you're ready, tap Submit at the bottom of the form. A confirmation will appear once your response has been recorded.
Note: Submissions cannot be edited once submitted. If you make an error, let your service Admin know.
Viewing your past submissions
You can view your own past submissions for any form directly in the app.
From the Forms screen, tap a form to open it, then tap My submissions to see a list of your previous responses. Tap any submission to view the full response you recorded.
Note: You can only view your own submissions in the app. If you're a service admin and need to view all submissions across your team, use the form manager on the Storypark website.
Frequently asked questions
Can I create or edit forms in the app?
No — creating, editing, and publishing forms is done on the Storypark website by Admin educators. The app is for completing forms and viewing your own submissions.
Can I upload a document to convert into a form using the app?
Not at this stage — the AI form conversion tool is available on the Storypark website only. Once a form has been created and published by an Admin, it's available to complete in the app.
Can I complete a form if I'm offline?
The app requires an internet connection to submit a form. If you lose connection mid-completion, stay on the screen — your progress will be held and submitted once you're reconnected.
Can I view submission records for my whole team in the app?
No — the full submission history for a form is only accessible to Admins on the Storypark website. In the app you can view your own past submissions only.
The form I need isn't showing in the app — why?
Only published forms are visible in the app. If a form you expect to see isn't there, check with your service Admin that it has been published. If Forms & Checklists isn't showing at all, it may not yet be enabled for your service.
I belong to multiple services — will I see forms for all of them?
Forms are shown for your currently selected service. To switch services, tap Me in the bottom menu and select your other service from the service switcher.
